If you're eligible for a refund of overpaid premiums for your OSHC, complete this form and attach the relevant documents. It may take up to 10 business days for your refund request to be processed.

Where will my refund be paid

We can pay your refund into the bank account we have saved for you (where your claims are paid), or to the credit or debit card used to purchase your policy. If the card used to purchase your policy is no longer valid, please select payment by bank transfer. If the card isn't in your name, you'll need to contact the owner to organise reimbursement. You can check or amend your bank details by going to Payments.

I acknowledge it is my responsibility to hold valid health insurance for the entire period that I am in Australia on a student visa and failure to do so can invalidate my visa status and affect any future visa applications.

I acknowledge that nib is required to provide my name, contact details and health insurance cancellation details to the Department of Home Affairs (DHA).

I give permission for nib to verify my current Visa status via the VEVO system for the purposes of confirming refund eligibility.

I acknowledge that: • The information I have provided in this form is accurate and complete. • nib may, at its discretion, contact me for further information. • Once my refund is processed, my policy will be cancelled and I will no longer have insurance coverage.
